PHYSICALS AND ANNUAL EXAMINATIONS
FOCUS ON PREVENTION AND SCREENING
The Pacific Northwest Wellness Center provides a variety of physicals and annual examinations for individuals and families. Our physicals offer a complete review of your health with the goal of prevention and screening. Physicals include:
Annual Physicals
Providing a complete review of your health status. In addition to a general exam, this program typically includes a complete blood count, a chemistry panel and a urine analysis.
School Athletic Physicals
Used to determine whether a student, athlete or camp attendee is medically and physically able to participate in the planned activities. A completed School Athletics Physical form must be on file with the school before students may participate in middle school or high school sports. Forms are available in our office or you may get forms directly from your school’s office or website.
Department of Transportation/Commercial Driver’s Licensing Medical Certifications
Government-mandated evaluations of the health status for commercial motor vehicle operators. The purpose is to detect the presence of physical, mental or emotional defects that would affect your ability to safely operate a commercial vehicle. All commercial drivers must meet minimum medical standards established by federal regulations. Drivers are required to have a valid physical examination card in their possession any time they’re operating a commercial vehicle. The Pacific Northwest Wellness Center provides a cost effective and convenient way to meet this legal requirement.
Pre-placement Employment Examinations
Document your general health and your ability to perform certain jobs. Employers covered by federal laws against disability discrimination (over 15 employees) may require a pre-placement physical examination to determine the suitability of an individual for a job. Learn more
